Alexander, 1945b: 27–29 (original description); 42, figure 11 (male terminalia). Ribeiro & Amorim, 2002: 5 (redescription), figure 4 (male terminalia); 4, figures 1 (head), 2 (thorax) and 3 (wing). Type locality: Brazil, São Paulo, Juquiá.
Holotype male with labels: (1) handwritten on white paper: “ Brazil | São Paulo | Juquiá | 400m Nov. 40 [November, 1940] | Travassos; (2) printed with handwritten inscriptions on red paper: “ HOLOTYPE ♂ | Elephantomyia | juquiensis | C. P. Alexander”.
Preservation: mounted at the tip of a pinned paper triangle; part of abdomen with terminalia mounted on a plastic slide pinned with the specimen. Missing parts: flagella, right wing, right legs, left front leg and left middle leg.
